Admission Application

We are currently accepting applications for the class beginning in January 2025.

Students applying for admission to Premier Diagnostic Institute should complete the initial application below. Once the initial application has been reviewed you will be sent a primary application with additional information. An application can also be obtained by calling or emailing the Premier Diagnostic Institute. Students must submit official college transcripts and two (2) recommendation letters to be considered. Students must submit the following to be considered for submission, application (along with $50 non-refundable application fee), official transcripts from all colleges/universities attended, two reference letters and current resume. Selected students are not determined to have a secured seat in the Program until the required Screening Interview, Background Check and Drug Screening are completed and acceptably scored. Students will be contacted regarding the decision on acceptance within 1 week of completing the screening interview and required application packet.
